The ultimate goal of the Flight Safety Department is to eliminate all aircraft accidents. Accident analysis has shown that the majority of all aircraft accidents can be traced to human failure. This human failure is often far removed from the direct operation of the aircraft. Therefore, the Flight Safety Program must include all aspects of human involvement in the operation of aircraft, and not just those which are visible.
